Frail Elder Program

The Frail Elder Program is a home and community-based service offered by Mass Health. It is to help elders in the community with personal assistance in their homes. This program is for people 65 and older, or people 60-64 with a disability,  living at home, and have both income and asset limitations. The application process is similar to applying for long-term care (nursing home coverage). Contact us today for more information!
